TC LIFTING CLUB, LLC RELEASE OF LIABILITY AND INDEMNITY

Emergency Contact:

To ensure the safety of our members, we require a waiver and release of liability. Please read the release carefully and provide your name, date of birth, address, phone number, and emergency contact information to complete this form. This document protects both you and The Compound Lifting Club from any potential injuries or accidents during your participation in our activities.

I, on behalf of my heirs’ successors, spouse and/or children, agree to release and discharge “THE COMPOUND LIFTING CLUB” from any and all liability, claims, demands, cause of action, and expenses (including attorney’s fees and costs). Whether past, present, future, for personal injury, death, and/or property damage from any cause whatsoever arising from or in connection with my use of “THE COMPOUND LIFTING CLUB” and/or participation in the activities, even if such injury, death, and/or property damage is caused by negligent act or omission by released parties.
